Care Coordinator - Home Care - £17k p/a + Enhancements

Care Coordinator/Administrator - £17000 + enhancements - West Molesey Area
An exciting opportunity has arisen and you will be coordinating home care workers in and around the area of West Molesey. We are are looking for a Care Coordinator to assist in the day to day running of a busy, national home care provider. You must have previous experience in care coordinating as you will be involved in direct line supervision of a team of home care workers. You will have ideally achieved a NVQ 2 - 3 in Care and have sound verbal and written communication skills with an understanding of CQC. You must have a driving licence and your own car, and have good local knowledge as visits to service users\\\\\\\' homes may be required. You may also be expected to be part of the oncall rota for which you will be paid extra. Extra money can also be earned working in the community.
Duties Include:
- To have a good understanding of the StaffPlan staff rota planning system or similar
- To Participate in the multi - disciplinary assessment of service user needs and the subsequent planning of service user care.
- To Identify special care needs and monitor and review as appropriate
- To liaise with GPs and Nursing professionals in respect of developing a total package of care for a service user.
- To work closely with the branch manager and report on activities and outcomes for service users.
- To be involved with staff development, training and appraisals and to ensure care staff are well supported and valued.
- To write reports on complaints, accidents and incidents as required.
- To operate day to day care services in line with the latest health and safety policies.
- To participate in meetings on all staff issues as required by the branch manager.
- To have business development skills & ability to plan staff rota\\\\\\\'s with care and attention
